Employee Is Killed When Struck by Front-End Loader

On October 14, 2004, Employee #1 was placing waste from a drain line into a front-end loader truck. The operator of the truck moved the loader forward, striking Employee #1. He was crushed between the bucket of the loader and a building wall. Employee #1 was killed.
