Investigation Summary
On July 28, 2005, Employee #1 was trouble shooting J Bar (Test Transmission Stand), he was working on stand #5 had it locked out and for some unknown reason he moved over to stand #4, Operator for #4 went to lunch had a floater fill in for him while he went to lunch, operator hit button to raise stand and employee was at the top of stand #4 and was crushed to death.
