Employee Killed When Raised Forklift Falls on Him

At approximately 9:00 a.m. on November 12, 2005, Employee #1 was attempting to replace a starter on a Hyster forklift. He used a forklift jack to raise the front of the truck in order to place a 6 in. by 6 in. block and a 2 in. by 4 in. piece of wood stacked on top of the block. He placed the stack under the left front tire and positioned himself under the left front frame of the forklift, about halfway between the front and rear tires. Employee #1 had removed the old starter and was installing the new starter when the forklift rolled backward off the wood blocks. His head was pinned between the frame and the concrete floor. A coworker, who realized that Employee #1 was unresponsive, ran to the front of the building and directed other employees to call an emergency response unit while he used the forklift jack to raise the serviced truck from the victim. Employee #1 was pronounced dead at the scene.
