Investigation Summary
At approximately 12:30 p.m. on July 21, 2005, Employee #1 had backed a tractor-trailer into a warehouse to unload bales of recycled paper. The trailer had a 9,000-lb loading ramp that was held in place during transport by a wire rope cable with a main pin placed through a clevis which is attached to the cable and ramp. On the end of the pin, there was a hole to place a secondary pin or clip to secure the ramp in the raised position, even if the vibration of the moving trailer caused the primary pin to fall out. Apparently, Employee #1 failed to use a secondary pin and the primary pin worked out of the clevis while the trailer was moving. When the trailer's rear doors were opened, the ramp fell, striking Employee #1, and pinning him under it. A skip loader was used to free Employee #1 and he was transported to St. James Olympia Hospital, where he died the next day of multiple crushing injuries.
