Investigation Summary
At 1:30 p.m. on August 26, 2004, Employee #1 was loading the bed of the work truck with landscaping clippings and trash. He was standing in the bed of the truck when he lost his balance due to the forward momentum of the truck. Employee #1 fell over the tailgate of the truck and was struck by and run over by the trailer the work truck was pulling. Employee #1 incurred severe head and chest injuries and later died from his injuries.
