Investigation Summary
At approximately 2:30 p.m. on May 2, 2005, an employee and coworkers were picking up a scissor lift from a construction site. The employee was acting as a guide for the coworker who was backing the flatbed truck into a loading dock. The loading dock was located at a building next door to the construction site where the company had been working the previous week. The company was in the process of picking up and loading a scissor lift that they had used at the construction site the previous week. To load the scissor lift onto the truck, the employees were going to use the dock which would have enabled them to drive the scissor lift from the dock onto the truck. The employee was standing in the dock area while acting as a guide for the coworker who was backing the truck into the dock. The employee was positioning and holding the dockleveler up to allow the truck to back in underneath it. According to the coworker, he was watching the employee out of his passenger mirror for guidance on how close he was to the dockleveler bumpers at the time of the accident. According to the coworker, the employee was motioning him to continue backing up until the last moment when he motioned for the him to stop. At that point it was too late and the truck crushed the employee against the dockleveler bumper. The employee was crushed between a dockleveler bumper and a flatbed truck, and was killed.
