Investigation Summary
At approximately 1:40 p.m. on December 16, 2003, an employee, a press operator, was feeding a 2-in. wide flat piece of steel into the point of operation equipped with a notching die that formed 45-degree angles on each corner of the flat steel. The point of operation was equipped with a guard; however, it was not fastened to the bolster plate thus allowing the operator the option of easily lifting the guard at her discretion. At the time of the accident, the employee inserted her fingers under the plate of steel for repositioning. The plate stood on its edge thus lifting the guard at the same time. The foot pedal was then depressed, activating the ram, causing the plate to slam back to its original position and catching the employee's index and middle fingers. The left index finger of the employee was amputated at the middle knuckle and the middle finger was smashed, however, 90 percent usage was regained. The employee was hospitalized.
