Investigation Summary
Employee #1 and a foreman for the prime carpentry contractor were using a chalk line and tape measure to measure and mark interior walls on an open-sided floor, four-stories tall. Employee #1 fell approximately 32 ft, feet-first, onto a pile of plywood, and snow. Employee #1 sustained a broken pelvis, and fractures to one leg, along with compressed vertebrae and discs in his back. There was no safety monitoring system, nor conventional fall protection in use.
