Investigation Summary

Investigation Nr: 200001915
Event: 11/20/2004
Employee Is Killed in Fall from Roof

On November 20, 2004, an employee, a bricklayer, and two coworkers, a bricklayer and a foreman, were working on the new North Kansas City Hospital Health Services Pavilion located in North Kansas City, Missouri. All three individuals were on the east side of the facility roof. The employee and another bricklayer had been working on suspended scaffolding and washing down new brick on the east wall of the facility. They had reached the point in the work where the suspended scaffolding needed to be moved from the location where it was laterally to the north on the east wall so the wash down could continue. The employee and another bricklayer were on the roof to move the scaffolding outriggers and associated equipment used to support the scaffolding. The employee was measuring along the east edge of the roof to determine the new location of the outrigger and associated equipment that was to be placed at approximately the northeast corner of the roof, when he fell over the north side of the northeast roof corner to the ground. He was killed.
