Employee Falls from Roof and Dies

At approximately 10:30 a.m. on January 20, 2006, Employee #1 and two coworkers were installing 0.625-in. plywood roof decking on a structural steel framework using screws. The framework was in the form of a board arch. The plywood was cut on a work table and lifted up to the work area with a rough terrain forklift. Employee #1 fell approximately 19 ft from the roof decking to the concrete slab below and died.
