Employee electrocuted installing limit switch on dishwasher

On October 25, 2005, Employee #1, a journeyman electrician, was installing wiring for a limit switch on an automatic dishwasher in a school cafeteria kitchen. His arm contacted the 380-volt tap on a multi-tap transformer inside the dishwasher, and his other arm contacted the metal cabinet of the dishwasher. Employee #1 was electrocuted.
