Investigation Summary
On April 21, 2005, workers were to remove an old punch press from their facility and install a new one. The Punch press was too large to take out through any doorways so an opening had to be made in the roof of the building. The opening was 12 ft by 20 ft, and two perlins were cut and removed to allow the punch press to be removed through the roof. The old punch press was removed and the new one installed through the roof. Employee #1 and a coworker had to re attach the perlins. They used an aerial lift inside the building to get up to the roof to weld the sections of the perlins back in place. Once they were in the roof opening where one of the perlin sections was to be attached, Employee #1 said he was going to stabilize the perlin in place. He stepped out of the basket and onto the roof. The roof of the building was corrugated steel panels and the skylight was corrugated fiberglass panels. It was difficult to distinguish between the corrugated steel panels and the fiber class panels. There was nothing to differentiate between the two panels. The skylight was not protected to prevent workers from walking onto it. Employee #1 stepped on a sky light and fell through. Employee #1 was hospitalized and died.
