Investigation Summary
On October 7, 2004, Employee #1, with Pizana Construction Inc., was with a crew a framing residence. After setting down a piece of lumber, Employee #1 turned and fell 10 ft through a stairwell opening. He struck his head on the concrete floor and was killed. Employee #1 was not using fall protection equipment, and there were no guardrails protecting the floor opening.
