Employee's arm is amputated by conveyor belt

On July 7, 2004, a pipe got stuck in an overhead conveyor belt. Employee #1, a supervisor, had one of his coworkers lift him up in the bucket of a skid steer to remove the pipe. The conveyor belt was not locked out and the tower operator started the conveyor belt. Employee #1's left arm was ripped out of his shoulder socket. He was hospitalized and treated for amputation.
