Fall from scaffold kills one employee and hospitalizes other

On August 27, 2004, two employees worked on a two-point suspended scaffold and fell approximately 40 feet. One employee was killed, and the other employee suffered serious injuries and was hospitalized. Neither employee was wearing a personal fall arrest system.
