Investigation Summary
Two employees were attempting to move a granite slab that was approximately 70 in. by 130 in. in size. Employee #1 was standing in front of the slab, waiting to attach the forklift clamp to the slab. The slab was approximately at a 90-degree angle. The slab went beyond 90-degrees, pinning crushing Employee #1 against a concrete center block wall. Employee #1 was killed.
