Employee is killed when struck by forklift

On August 19, 2004, Employee #1, a mechanic, was standing directly behind a forklift getting the hour meter reading. The forklift operator had left the forklift running when he dismounted it to test the lights. He engaged the reverse as he climbed back into the forklift. The forklift backed over Employee #1, killing him.
