Investigation Summary

At approximately 10:00 a.m. on August 9, 2004, Employee #1 was repairing a cable TV connection mounted approximately 12 ft high on a utility pole. He was working out of a uninsulated Altec bucket, Model Number AT200AV, mounted on a utility van. He drove the vehicle into the bushes surrounding the site and proceeded to work. He had to return to the ground, but to do this he had to raise the bucket over the back of the van and lower it to the rear of the vehicle. He raised the bucket into power lines and contacted a 13,200 volt conductor. He suffered severe burns to the lower portion of the body and was hospitalized.
