Employee is killed, another injured in fall from forklift

On September 1, 2004, Employees #1 and #2 were lifted in a job-fabricated man-basket or personnel platform on the forks of a Lull brand rough terrain lift. The basket was not secured to the forks, or otherwise attached to lift. When the basket was elevated to a height of approximately 22 ft to 25 ft, the basket tipped and fell to the ground, dumping out the two employees. Employee #1 suffered a fatal head wound. Employee #2 survived the fall, suffering only a fracture of the pubic ramus.
