Investigation Summary
At approximately 2:45 p.m. on June 1, 2004, Employee #1, a home improvement worker and roofer, was installing vinyl-siding at the rear of a two-story residential house. He was using two 20-foot aluminum ladders, interchangeably, that were propped up against the house and their feet placed on a grassy surface. While he was installing the last four courses of vinyl siding, a thunder storm was approaching and he rushed to complete the job before the weather changed. A strong gust of wind hit him, causing the ladder he was on to shift about 12 inches to the left and the ladder buckled and collapsed. Employee #1 fell 20 feet to the ground. He was initially transported to Carroll Hospital Center and then later flown to Union Memorial Hospital for treatment and observation. He suffered a sprained left knee. A detailed investigation revealed that the combination of Employee #1's weight of 220 lbs and the poor condition of the ladder were the root causes of the accident. The aluminum ladder that had collapsed had a usable length of 19 feet, was very old and badly corroded with significantly bowed rails. Additionally, the ladder had a broken support bracket dog on the right side and the bottom left rail was cracked and bent inward at a 45-degree angle. The rope and hardware that allows the user to extend the ladder to its desired length from the ground were also missing. Both sections of the extension ladder were separated at the time of the inspection. The residential building owner stated she had requested that the employer, who was working from the same ladder two days prior, to replace it. Further, Employee #1 stated that he may have overextended the ladder past its standard intended length in an attempt to reach the eve of the roof that was 23-feet high.
