Employee Dies from Unwitnessed Fall

At approximately 12:30 p.m. on August 17, 2005, Employee #1 was in the process of gathering his work materials after finishing a wall. According to a coworker who was working in the area, he was talking to Employee #1, finished his conversation, and turned to walk away, when he heard something fall. The coworker turned back around and saw Employee #1 lying on the concrete ground, having a seizure. He was lying next to a 5-ft scaffold, but the coworker did not see him on the scaffold or see him fall from the scaffold. Employee #1 died from a concussion to his head.
