Employee crushed under rough terrain container handler truck

On May 22, 2004, an employee of a subcontractor was in the process of setting up for changing the right front tire on a Rough Terrain Container Handler Truck, RTCHT. The employee was under the RTCHT positioning some additional wooden blocks used to support the vehicle, when the wooden blocks supporting the right side of the vehicle failed. The employee was crushed between the frame of the RTCHT and the wooden blocks at the head and shoulders.
