Investigation Summary
On June 29, 2004, Employee #1, a fork lift operator, was working in a polystyrene manufacturing plant. He punctured a hole in the bottom bag of a "super sack" of expandable polymers stacked two high. After moving the forklift out of the way, he was attempting to patch the leak, when the upper 2,205 pound bag fell. Employee #1 was crushed to death.
