Investigation Summary

Investigation Nr: 200623056
Event: 07/07/2004
Employee is killed when run over

On July 7, 2004, a road crew, consisting of four men, was in the process of backing up a milling machine and water truck in order to dig up a final 6-foot wide section of asphalt near a major roadway interchange. The water truck contains 4,000 gallons of water and is used in conjunction with the milling machine to fill a water reservoir for the milling machine at various times during the milling process to cool the milling teeth. For some reason, Employee #1 stepped into the path of the water truck as it was backing up and was knocked down and run over by the passenger side rear dual tires. Employee #1 was killed.
