Investigation Summary
At approximately 7:15 p.m. on May 3, 2005, an employee involved in a paving operation was thrown from, and then crushed and killed by, an Ingersoll Rand, PT 125R, Pneumatic Roller. The employee was apparently crushed by his machine, when the machine ran off the road and rolled down an embankment.
