Employee is killed in fall from ladder while painting

Employee #1 was painting the upper part of a garage from a 28-foot extension ladder when he fell approximately 9 to15 feet onto the concrete driveway. Employee #1 struck his head on the concrete and was killed. The ladder remained in the upright position.
