Investigation Summary
On April 19, 2004, an employee was removing ice from the salad bar. He was putting the ice in a wheeled "ice tote" and taking it into the kitchen area for disposal. On the employee's first trip into the kitchen, he spilled some ice on the floor, but did not clean it up. On employee's second trip, he slipped on the water from the ice that had melted and struck his head on the ice tote. The employee suffered a small laceration above the right eye. He was not hospitalized.
