Employee Is Brain Dead in Fall from Ladder

On February 7, 2006, an employee fell approximately 7 ft, when the portable metal ladder stand that he was using tipped over. The employee suffered severe head trauma and was later pronounced brain dead. A wheel was missing from the ladder at the time of the inspection.
