Employee is injured when fingers crushed in press

On August 21, 2004, Employee #1 was adjusting the rollers and brushes in the folder and chopper box of a press as it ran at thread speed. He was making adjustments to the thumbwheel on the right side of the folder and chopper arm and was preparing to adjust the thumbwheel on the left side when he reached between the arm and the guard that covered the rotating counter balance. The counterbalance was rotating in a counter-clockwise motion and cycled down on his thumb and index finger. A coworker hit the emergency stop button and cycled the folder and chopper forward so that it would release Employee #1's hand. His thumb and finger were partially amputated and he was hospitalized.
