Investigation Summary
Two employees were lifting a 168 pound HVAC condensing unit up to the roof of Oakdale Middle School. They were each using two 0.5-ton rope block and tackles to pull the condensing unit up to the roof. They were also using a Type 1A fiberglass extension ladder as a ramp to keep the condensing unit away from the brick wall. Employee #1, the supervisor, was standing at the bottom of the ladder observing. He noticed that the box was shifting to the side so he went up the ladder to steady it. Employee #1 stated that he weighed approximately 200 pounds and the maximum working load for the ladder was 300 pounds. As the condensing unit was lifted off of the ladder, the ladder shifted to one side and Employee #1 tried to pull it in the opposite direction with his weight. He overcompensated and the ladder started to fall. As the ladder started to fall, Employee #1 jumped clear of it so that he did not ride the ladder down. The employees on the roof grabbed for the ladder and Employee #1, only catching the ladder. Employee #1 stated that for heavier loads he usually requested a small crane truck or a lift truck. Employee #1 fell approximately 18 feet, 3 inches and fractured both legs as a result of the fall. Employee #1 was hospitalized for his injuries.
