Investigation Summary
On April 1, 2004, Employee #1 and five coworkers were securing metal decking on the roof of a building that was under construction. Employee #1 and his coworkers were located in a controlled decking zone that was put in place by their employer. One of Employee #1's coworkers asked for assistance in repositioning a 31-foot-long section of metal decking that had started sliding out of place during the securing of it. As Employee #1 and his coworker attempted to reposition the metal decking, Employee #1 walked forward into an opening approximately 3-feet wide, which was created when he picked up the section. Employee #1 fell approximately 28 feet to the ground. He was hospitalized and treated for multiple injuries to include: lacerations to his face, a broken jaw, and compound fractures to both arms and legs.
