Investigation Summary
On October 27, 2004, Employee #1, the end dump helper, was on the ground in front of the end dump conveyor during the process of unloading sugar beets from a truck on the end dump side of piler #2. Sometime, probably less than one minute, after the last of the beets were out of the truck and on their way to the pile, Employee #1 made contact with the tail end of the conveyor, was pulled into or fell onto the conveyor, and was pulled into the conveyor system and killed. This area had an expanded metal guard that was tied with wire rope in the 'up' position, for a duration of at least two campaigns.
