Employee Is Killed When Fell from the Ladder

On April 12, 2004, an employee was working from a ladder, approximately 11-ft. in length, installing a Valance above a stage when a rear support leg of the ladder gave way, causing him to fall. Witnesses place him on the second or third rung from the top, at the time of the accident. The ladder was made of fiberglass, and it appeared that both rear support legs had been previously wrapped with white Dupont Vinyl Tape. The employee selected the ladder from a stack of ladders left at the site by other sub-contractors to the General Contractor.
