Investigation Summary
On January 1, 2008, Employee #1 was loading bagged processed fish onto a horizontal conveyor within the freezer hold of an F/V Siberian Sea Vessel. The vessel has a type of conveyor that has a flat running surface which is interspersed with paddles in which the conveyed product is deposited and elevated out of the freezer hold to the main decking for transfer to the tramper. The conveyor deposited the product onto a vertical conveyor or "box ladder." The box ladder was shutdown to allow the deck crew to catch up and transfer the main deck-product that had accumulated. As Employee #1 cleared accumulated ice or fish bags from around the base of the box, the ladder penetrated the freezer decking on its return rotation into the freezer hold. The box ladder was activated to resume the product transfer. A paddle from the rotating box ladder struck Employee #1 on the head from behind as the box ladder followed a vertical track to the freezer deck, killing the employee. The operator of the box ladder was on the main deck and did not have visual contact with the employee or anyone else working in the freezer hold.
