Employee Dies after Fall Down Elevator Shaft

On November 21, 2005, Employee #1 was part of a three-person team, using a flat bed cart and a forklift to move a 2,000 lb load of bagged ice, from the ground level to the suite level of a sports stadium. The forklift was being used to push the loaded flat bed up a small incline, to the entrance of the service elevator. Employee #1 was guiding the cart from the front. Employee #1 and the forklift operator did not realize that the elevator had departed the ground floor. The forklift continued to push the load of ice forward and into the closed elevator shaft doors. The forklift pushed the doors off of their bottom tracks, and pushed Employee #1 into the elevator shaft. He was injured, when he fell approximately 31 ft to the base of the shaft. He was hospitalized for head trauma, and died on December 05, 2005.
