Investigation Summary
At about 11:15 a.m. on February 22, 2005, Employee #1, of Peak USA, was dismantling and lowering the top substructures of an oil drilling rig. The 80-ton crane used to lift the substructures began to tilt forward and lose the load. Employee #1 was standing next to the front right track of the crane. When the load of substructures struck the ground, the boom of the crane collapsed, elbowed out, and struck him. Employee #1 received massive traumatic injuries to the upper part of his body, including amputation, and was killed.
