Investigation Summary
On January 3, 2005, Employee #1, a cargo truck driver, was preparing cargo for discarding. The employee was struck and crushed by a powered industrial truck (PIT) that was backing up. Employee #1 was killed. The PIT Operator said he did not see the employee before the impact into the side of the cargo truck.
