One employee dies and another injured from fall

On June 2, 2004, Employees #1 and #2, who were father and son owners of a painting business, were standing on a fabricated metal platform to paint an existing two-story residential building. The metal platform was placed on the highest level of a three tiered scaffold tower that was erected three feet from the exterior wall of the building. This positioned Employees #1 and #2 working above the buildings roof tiles that had a 10- to 12-foot pitch. For some reason, the scaffolding collapsed and both men fell. Employees #1 and #2 were hospitalized. Employee #1 died and Employee #2 was treated for multiple contusions and abrasions.
