Employee's Leg Is Bruised in Palletizing Machine

On March 4, 2004, an employee entered a palletizing machine to clear a jam without using a lockout and tagout procedure. The machine suddenly started up catching his legs in between a moving metal bar and the floor. The employee's lower leg was bruised but he was not hospitalized.
