Investigation Summary
At approximately 9:30 a.m. on November 17, 2006, Employee #1 was clearing jammed concrete blocks in a belt conveyor. He was pulling on the belt for the conveyor motor, LONGEM, Model Number TM4215010, 10 hp, 1745 rpm, by using both of his hands. He was injured when a coworker turned the conveyor on, and Employee #1's hands became caught in the belt and pulley of the motor. He was hospitalized for the amputation of his right ring finger and multiple fractures on both hands which required two surgeries.
