Two Employees Struck by Falling Concrete, Suffer Amputations

At approximately 10:00 a.m. on October 10, 2006, Employee #1 was removing a mold from a precasted concrete trash receptacle (Model Number S-TR-A, weight 900 lbs, 36-in. high and 22-ft wide). The receptacle was hanging from an overhead crane approximately two feet above the ground. The receptacle was rigged using hooks and chains. The hooks were attached to two lifting loops casted inside the walls of the container. One of the lifting loops failed (metal wires broke off) and one side of the receptacle fell on Employee #1's right foot and causing the amputation of three toes, Employee #1 was hospitalized. Employee #2 came to help Employee #1 and tried to lift the receptacle with his hands. The receptacle fell on Employee #2's right hand which caused the amputation of Employee #2's right middle finger. Employee #2 was also hospitalized.
