Employee Is Entangled in Forklift Mast, Crushes Forearm

At approximately 11:40 a.m. on July 27, 2006, Employee #1 was operating a forklift, when he became entangled in its mast. His left forearm was crushed, and he was transported to Loma Linda University Medical Center. Employee #1 underwent surgery and was hospitalized for four days.
