Investigation Summary
At approximately 2:30 p.m. on August 01, 2006, Employee #1 was cutting wood with a Jet Table Saw, Serial Number 88 at a facility that manufactured finished doors. The blade of the table saw was unguarded. As he cut the wood, his finger contacted the blade, amputating it. He was transported to a medical center. At the hospital, he received medical attention for the injury and then released.
