Investigation Summary
At approximately 6:45 a.m. on August 14, 2006, Employee #1, a Field Supervisor, got his Ford F-450 Crew Cab truck stuck while crossing a swale. Employee #1 was lying down between the rear wheels and back bumper digging dirt out from under the trailer hitch. A laborer then got into the cab, started the truck, put the transmission in reverse causing the truck's rear wheels to run over Employee #1. Employee #1 did not authorize, and was unaware of, the laborer's action. Employee #1 suffered catastrophic fractures to his right shoulder chest and head and was hospitalized for 30 days.
