Investigation Summary

Investigation Nr: 202443958
Event: 05/24/2006
Employee Is Struck by Hose and Falls Off Platform

At approximately 1:15 p.m. on May 24, 2006, Employee #1 was standing on a steel platform of a mixer truck. He was using a garden hose to wash mixing fins inside the drum of the truck. Adjacent to the mixer truck, there was a concrete pump truck. The hose of the concrete pump truck was attached to the hopper of the mixer truck. The hopper was filled with water, which was used to clean the concrete hose. As the concrete hose was undergoing cleaning, it plugged and whipped violently. The hose struck Employee #1, knocking him from the platform. Employee #1 fell 8.42 ft onto the ground, and sustained fractured ribs and punctured lung. He was transported to a medical center. At the hospital, he underwent a medical evaluation and received medical attention for his injuries. Following the medical procedures, he was hospitalized for eight days.
