Investigation Summary
At approximately 1:30 p.m. on March 29, 2006, Employee #1 was working for a processor and distributor of grain foods. He was making an adjustment on the bagging mechanism on a Slidell Model Number Slidell-Matic 3320 packaging machine. Just before the accident, the operator of the machine told Employee #1 that the machine's spout was tearing the bags. Employee #1 noticed that the bag was not coming out correctly from the supply stack. He stopped the machine by pushing the emergency button and opened the side doors to access the bag guide. As he made an adjustment on the bag guide, the lift chair for the bag came up, pinning Employee #1's upper body between the chair and the bagging mechanism. A coworker other than the machine's operator closed the doors and reactivated the machine to release Employee #1. Employee #1 sustained a skull fracture, broken upper left jaw, collapsed lungs, laceration on his left jaw near the ear, and laceration on his left upper arm. He was hospitalized for 6 days. He was a regular employee of the employer.
