Investigation Summary
At approximately 8:30 a.m. on February 17, 2006, Employee #1 was walking down a ditch about 3 ft deep. The employee and three other coworkers were at the site to clear out the storm drains by using shovels. They had not started work yet. The employee walked down the ditch, where she slipped and fell. Her left ankle got twisted. Employee #1 suffered two fractures on her left foot, which required surgery and two days of hospitalization. The employee was a temporary employee of the employer.
