Investigation Summary
At approximately 12:51 p.m. on December 29, 2005, Employee #1 was operating a Skilsaw. As he cut a piece of plastic, his thumb contacted the blade, partially amputating it. Employee #1 was transported to a medical center where surgery was performed and the severed portion of the thumb was reattached. After surgery, he was hospitalized for one day.
