Investigation Summary
At approximately 6:30 a.m. on December 19, 2005, Employee #1, a roofer, was on a panelized roof approximately 4 feet from the edge and helped remove a generator from a Gradall forklift, Model Number 534D-9 # 702, Serial Number 0544440 and set it on the roof. As the forklift operator retracted the forks of the Gradall, the bottom of the mast caught and pulled the leading-edge truss causing the 50-ft by 8-ft panel on which Employee #1 was standing to fall. The panel and Employee #1 fell approximately 40 ft to the concrete floor. Employee #1 was hospitalized for treatment of multiple fractures.
