Investigation Summary
At approximately 8:00 p.m. on November 29, 2005, Employee #1 was unloading a pallet of 18-in long rebars from a flatbed truck, using a forklift (Clark-lift, Model Number C5-Y580, Serial Number Y685-3676665K0F). Employee #1 drove the forklift towards the side of the flat bed truck before he picked up the load and got off the fork-lift. When a coworker went to pick up chain from another truck (Bob Tail), he saw Employee #1 pinned between the forklift forks and the side of the flat bed truck, underneath the bed. There were no witnesses to the accident. Employee #1 was hospitalized for a chest injury. He died at the hospital several days later, on December 15, 2005.
